Assistant Medical Director (Erie): Under the direction of our Medical Director, this position leads the primary care team to ensure the fulfillment of the One Senior Care Mission, Vision, Values and Goals. Using a collaborative approach, advances, influences, and implements forward thinking clinical practices. Providing general medical direction and leadership for all aspects of medical care – including coaching, guiding, advising, and educating the clinical teams.
Assures the most appropriate, effective utilization of medical care to ensure the delivery of high-quality participant services. Provides clinical care services to participants as indicated.
Occupational Therapist (Warren): our Occupational Therapists are responsible for planning and delivering care for assigned treatment programs. This role participates in the interdisciplinary rehabilitation team within a post-acute rehabilitation environment and all group programs provided by the rehabilitation services department.
Activities Coordinator (Erie, Meadville): Our Activities Coordinators plan, direct and carry out a program of activities that provide opportunities to enhance optimal levels of physical, cognitive, and social functioning (i.e., exercise, active/mental games, music, crafts, etc.) for elderly seniors.
Driver / PCA (Mercer, Meadville, Erie), Our Drivers play a crucial role in the care of our participants as they provide transportation to and from their destinations. Driver/PCAs must want to make a difference in seniors’ lives by working with the interdisciplinary care team to implement and carry out services and activities to proactively meet the needs of our participants.
Personal Care Aide (Crawford, Mercer, Warren) our PCA’s play a crucial role in the care of our participants as they provide personal care and assistance in the center or from the comfort of their homes. PCA’s must want to make a difference in seniors’ lives by working with the interdisciplinary care team to implement and carry out services and activities to proactively meet the needs of our participants.
